| Appointments |
| Appointments are necessary for all patients. If you cannot
come at your appointed time and need to reschedule, please call the
office as soon as possible - preferably 24 hours in advance. Every
attempt will be made by the physician to see you at your appointed
time. However, unforeseen emergencies may arise, and for this
reason a delay in your scheduled time may occur. |

| Payment
Options |
| Payment for office services is requested at the time services
are rendered unless other arrangements have been made in advance. An
itemized statement will be provided to you at the time of your visit
to aid you in seeking reimbursement from your insurance company. Our
staff will be happy to answer any questions you have regarding our
fees for service, or insurance filing or reimbursement. For
your convenience VISA and MASTERCARD are accepted for payment. We
do not accept Medicare assignment on office visits. |

| Telephone
Policies |
| Telephone Policies are meant to allow us to care for our patients
with a minimum of interruptions. Telephone messages will be
taken by the receptionist and relayed to the doctor. Your call
will then be returned. Please call during regular office hours
whenever possible. It is easier to manage a problem when the
records are immediately available. |

| Emergencies |
| For Neurological emergencies call our office. After
hours or on weekends, the answering service will take your call,
and will
contact the doctor who will return your call. |

| Prescriptions
and Refills |
| Prescriptions and refills are generally handled during office
visits. For telephone refills, please have available the name
of the medication, the prescription number, and the pharmacy's telephone
number. Prescriptions will only be refilled for patients actively
under our care. Please attempt to limit requests to office
hours. |

| Hospitalization |
| Hospitalization may be necessary to properly manage certain conditions. Our
office will make the necessary arrangements. Should it be necessary
for you to change your admission date, please contact this office
first. |

| Services |
| Physician service at the hospital will be itemized and billed to
you by mail. As a service to you, our insurance secretary will
file your insurance for physician services at the hospital. These
charges are separate from those made by the hospital. The insurance
claim may go directly to your insurance company through the mail
or by computer, or you may be sent a claim form with special instructions
which we ask you to sign and mail yourself.
We will wait a reasonable length of time for your insurance to pay
(usually 30 to 60 days) and then ask that you begin making regular
payments. Please remember with most health insurance the responsibility
for payment of bills rests with you, the patient. Even
the best insurance generally only pays part of any doctor's fees. It
is your responsibility to collect from your insurance company or negotiate
a settlement on your insurance claim. An unresolved personal
injury claim (for example an auto accident) is not a reason for delayed
payment. |
